Site Registration

1) Click on the DOWNLOAD XLSX below to download the site registration form

2) Complete the site registration form and save it as SRF - [Your Name] - dd mm yyyy. Please do not save the file as a PDF. Save it as an Excel file.

3) Take a photo of the back and front of your CSR card and save it as:

CSRFront - [Your Name] - dd mm yyyy

and

CSRBack - [Your Name] - dd mm yyyy

4) Attach the Site Registration Form that you just completed along with the 2 images of your CSR card - front and back of the card.

5) When everything has been attached click on the Send Details button.
